Job Summary
Provide social services to patients in the home and provide clinical direction to fellow Social Workers and other clinical staff. Manage a caseload as an MSW and assist fellow MSWs by guiding patient issues and connecting to community and financial resources. Ensure appropriate cross-referrals within designated regions, assist in staff orientation, and may supervise Social Work Assistants and co-sign orders. Coordinate guest speakers and in-services for the Social Work Department, assist with in-house presentations, and develop/maintain strong community outreach on behalf of HCR Home Care. Office-based with light physical requirements; includes Mon-Fri days and a 40-hour work week. Requires a Master’s degree (CSWE-accredited) and licensure as LMSW/LCSW in NYS, with a minimum of 2 years’ medical social work experience.
Required Qualifications
- Master’s degree from a school accredited by the Council of Social Work Education
- Licensed Master of Social Work (LMSW)/Licensed Clinical Social Worker (LCSW) in NYS
- Minimum 2 years’ experience in medical social services setting
