MEDICAL SOCIAL WORKER I

Assist patients, families, and significant others with physical, emotional, and social concerns during hospital stays. Provide discharge planning services and complete required documentation prerequisites prior to discharge. Conduct initial assessments and continued documentation to obtain pertinent background, social, and financial data, including social determinants of health. Connect patients with community resources to aid treatment and recovery, and act as a facility liaison with adult and children protective services. Cooperate with hospital staff and physicians by informing them of patient progress through verbal communication and chart documentation. Follow policies, procedures, and safety standards while completing required annual education assignments and participating in quality improvement initiatives.
