Job Summary
Advocate for patients and families by gaining access to resources during bereavement, including grief counseling and mental health services. Assess patients and families to help them cope with emotional issues and practical arrangements related to a diagnosis while exercising sound judgment. Ensure privacy and safety by adhering to local, state, and federal regulations and protecting protected health information. Collaborate with healthcare providers and peers to achieve departmental objectives while working independently within a team environment. Perform specific procedures and teaching duties as needed.
Required Qualifications
- Master's: Social Work or related field OR Masters of Social Services, MSS, degree from a school accredited by the Council on Social Work Education
- 2 years recent relevant experience
- BLS-Basic Life Support
- INS-Automobile Insurance DL-Valid Drivers License
- In depth understanding of the implications of illness, hospice and/or home care death and the dynamics of grief
- Ability to interpret a variety of data and instructions, furnished in written, oral, diagram, or schedule form
- Demonstrates working knowledge of resources and treatment centers including mental health and substance abuse services and facilities
- Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people
- Well-developed time management and organizational skills, including the ability to prioritize assignments and work within standardized operating procedures and scientific methods to achieve objectives and meet deadlines
- General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), electronic health records (EHR)
- Ensure the privacy of each patient's protected health information (PHI)
- Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions
- Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options
