Job Summary

Coordinate care with the Interdisciplinary Group/Team and participate in on-call rotation based on patient psychosocial assessments and family needs. Conduct comprehensive assessments within five days of hospice election to identify financial, environmental, and community requirements, while assisting with advance directives, funeral planning, and discharge planning. Provide counseling to patients and families regarding end-of-life issues, grief, and death, and collaborate on patient care planning to promote hospice philosophy and administrative policies. Maintain competency through mandatory continuing education, infection control policy adherence, and participation in the QAPI program. Work in patients' homes under flexible scheduling with local travel requirements.

Required Qualifications

  • Master's Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education
  • Baccalaureate degree in social work (BSW) from an institution accredited by the Council on Social Work Education
  • Baccalaureate degree in psychology, sociology, or other field related to social work and is supervised by a MSW
  • One year of social work experience in a healthcare setting
  • Baccalaureate degree from a school of social work accredited by the Council on Social Work Education
  • Employed by the Hospice before December 2, 2008
  • Not required to be supervised by a MSW
  • Current and valid license as a LMSW or LBSW issued by the Texas State Board of Social Worker Examiners
  • Current drivers' license
  • Reliable transportation
  • Valid auto liability insurance
  • Ability to work a flexible schedule
  • Ability to travel locally
  • Prolonged standing and walking
  • Ability to work under stressful conditions
  • Working knowledge of community resources
  • Good interpersonal skills
  • Ability to work with computers
  • Use effective interpersonal communication skills, both verbal and written
  • Meet mandatory continuing education requirements of the Hospice and licensing board
  • Maintain a working knowledge of public and private eligibility standards and requirements
  • Demonstrate commitment, professional growth, and competency
  • Follow the Hospice infection control policy
  • Participate in the Hospice's educational in-service programs as required

Desired Qualifications

  • One year of experience in palliative care or hospice preferred
