Job Summary
The Medical Social Worker provides psychosocial assessments and participates in care planning to support the social and emotional well-being of patients living with kidney disease and undergoing dialysis. Educates patients on treatment, health, and wellness options, and manages services to help patients cope with chronic kidney disease and dialysis treatments, collaborating with a multidisciplinary team to deliver holistic, patient-centered care that aims to improve quality of life.
Required Qualifications
- Master’s Degree in Social Work from an accredited CSWE institution
- MSW required
- LCSW/ACSW required
- LCSW preferred (CA)
- Licensure/LMSW required in states with licensure (TX, TN)
