Job Summary
Medical Social Worker – Home Therapies role focused on educating and supporting patients living with kidney disease and dialysis, conducting psychosocial assessments, participating in care planning, and managing services to enhance patients’ social and emotional well-being. Works with a multidisciplinary team to address psychosocial needs and improve quality of life for individuals undergoing kidney disease treatment. Required Qualifications
- Master's Degree in Social Work from an accredited CSWE institution
- MSW required
- LCSW/ACSW required, LCSW preferred (CA)
- Licensure/LMSW required in states with licensure (TX, TN)
- One (1) year of work experience in a behavioral or healthcare-related setting, preferred.
