Job Summary
Medical Social Worker - Home Therapies provides psychosocial assessments, participates in care planning, and supports the social and emotional well-being of patients living with kidney disease and undergoing dialysis. Works with a multidisciplinary team to educate patients on treatment options, enhance quality of life, and address psychosocial needs related to chronic kidney disease. Requires a Master’s in Social Work (CSWE-accredited); MSW is required, with LCSW/ACSW (LCSW preferred in CA) and LMSW licensure where applicable. Required Qualifications
- Master's Degree in Social Work from an accredited CSWE institution
- MSW required
- LCSW/ACSW required, LCSW preferred (CA)
- Licensure/LMSW required in states with licensure (TX, TN)
- One (1) year of work experience in a behavioral or healthcare-related setting, preferred
