Job Summary
Medical Social Worker position in Home Health/Hospice across California. Responsible for psychosocial assessments of patients and families, crisis intervention, and development of care plans with the interdisciplinary team. Conducts family and caregiver counseling, educates on advanced directives, maintains clinical records in EMR, coordinates information and referrals with community agencies, and supports end-of-life planning. Requires MSW (CA), CA driver’s license, CPR, and healthcare/hospice experience; LCSW preferred. Per diem, on-site in Ontario, CA with in-person duties.
Required Qualifications
- Master of Social Work (MSW) from an accredited school
- California driver’s license
- CPR Certification
- Minimum of one year healthcare or hospice experience
- Ability to conduct psychosocial assessments and provide crisis intervention
- Experience with EMR documentation
- Knowledge of Advanced Directives and funeral arrangements
- LCSW (preferred)
Desired Qualifications
- Master of Social Work (MSW) from an accredited program
- California LCSW license (preferred)
- CPR Certification (required)
- Valid California Driver’s License
- Minimum of one year of healthcare or hospice experience
- Ability to sit/stand for extended periods and lift up to 50 pounds as needed
