Job Summary
Conduct comprehensive assessments of clients' psychological, social, and medical care needs, coping mechanisms, home situations, and financial resources. Participate in developing and revising client Plans of Care while collaborating with families to meet social and medical goals. Maintain accurate, timely documentation and engage in case conferences, in-service programs, and discharge planning. Serve clients in Santa Barbara and surrounding areas with flexible daily pay options and weekly/daily schedule flexibility.
Required Qualifications
- Graduate of an accredited college with a Master of Social Work degree or equivalent
- Currently licensed in the State of California
- Completion of a minimum of one year of supervised work experience or the equivalent work experience in the field of psychology or sociology
Desired Qualifications
- Experience in a home health care setting preferred
