Job Summary
Per diem Medical Social Worker in Home Health visiting clients across Indianapolis. Responsibilities include conducting social evaluations, planning interventions, identifying community resources, advocating for clients and families, collaborating with a multidisciplinary team, developing care plans, maintaining contracts with agencies, educating patients and families about resources, and documenting observations and interventions in an EMR. Requires Indiana LMSW or LISW licensure and a Master of Social Work degree, with at least 2 years of relevant community-based experience. Travel to cases is expected; strong communication skills and computer literacy are essential.
Required Qualifications
- Must be licensed by the Indiana Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.
- A minimum of a Masters in Social Work, with at least 2 years of community-based experience.
- Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree.
