Job Summary
Conduct home health visits to clients in designated Salem County territories, assessing social and emotional factors related to health problems. Provide clinical social evaluations, plan interventions, and identify community resource referrals to address practical and environmental needs. Act as an advocate for clients and families to navigate the community system while developing total plans of care with the multidisciplinary team. Maintain contracts with public and private agencies and educate patients on community resources to promote independent living. Document all observations, interventions, and evaluations using a touch pad tablet. Requires NJ Board of Social Work Examiners Master or Independent licensure, a Master's degree, and two years of community-based experience. Base pay ranges from $85,800 to $93,600 annually.
Required Qualifications
- Licensed by the NJ Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level
- Masters in Social Work
- Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree
- Minimum of 2 years of community-based experience
- Ability to read, write, and effectively communicate in English
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases, as assigned
Desired Qualifications
- Home care experience a plus
- Prior experience with electronic medical records (EMR) preferred
