Job Summary
Conduct home visits to clients in Lamoille, Addison, Washington, and Caledonia counties to assess social and emotional factors affecting health. Provide clinical social evaluations, plan interventions, and identify community resource referrals to address practical and environmental needs. Act as an advocate for clients and families to navigate community systems while participating in multidisciplinary total plan of care development. Educate patients and family members on community resources to promote health, safety, and independent living. Accurately document observations, interventions, and evaluations using a touch pad tablet. Maintain contracts with public and private agencies as patient resources.
Required Qualifications
- Licensed Medical Social Worker, MSW
- Minimum of a Masters in Social Work
- Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree
- At least 2 years of community-based experience
- Demonstrated ability to read, write, and effectively communicate in English
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases, as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
