Medical Social Worker - Home Care

Educate patients on treatment, health, and wellness choices for chronic kidney disease and dialysis to enhance quality of life. Conduct psychosocial assessments, participate in care planning and team reviews of patient needs, and manage services to assist patients in coping with associated issues. Support the social and emotional well-being of patients at Satellite Healthcare while building relationships and serving as a servant leader to improve the environment for patients and staff. Master's degree in Social Work and required licensure are prerequisites for this role.
