Job Summary
Provide psychosocial/emotional assessments, care planning, and community resource referrals to patients and families referred to Halifax Health. Deliver clinical services centered on pediatric, adolescent, adult, and geriatric populations while managing crisis interventions and supporting team members across all organizational levels. Maintain strong communication skills with physicians, ancillary personnel, and colleagues to ensure patient/family-centered care. Requires a Master's Degree in Social Work (MSW) from a CSWE-accredited school, one year of healthcare social work experience, and a valid driving license.
Required Qualifications
- Masters Degree in Social Work (MSW) from Council on Social Work Education (CSWE) accredited school
- Valid driving license and good driving record
- One (1) year of healthcare experience
- One year of social work experience in a health care setting
- Working knowledge of systems theory and family dynamics
- Strong clinical and assessment skills, including crisis intervention
- Ability to work in a team setting
- Customer-service oriented
- Ability to effectively communicate and build relationships with Team Members at all levels in the organization
- Professionalism in interpersonal verbal and written communication skills
- Skilled in providing care for pediatric, adolescent, adult, and geriatric patients
