Medical Social Worker - Full Time

Conduct in-depth client assessments covering medical, psychosocial, environmental, financial, legal, and family domains. Perform Fall Risk Assessments, Mini Mental Status Examinations, and Depression Screenings for each client. Coordinate service needs and collaborate with hospitals, nursing homes, physicians, attorneys, and financial planners to implement care plans. Provide ongoing emotional support, crisis intervention, and problem-solving assistance to clients and families. Perform regular home visits to monitor status and environments while tracking time for accurate billing. Work with the business development team to drive referrals and attend educational networking events. Requires a Licensed Social Worker with 7-10 years of geriatric experience, a valid driver's license, and a dependable vehicle. Hybrid schedule with flexible hours.
