Medical Social Worker, Full Time

Job Summary
Provide services to patients, their families, or significant others facing social, economic, or environmental problems related to illness. Collaborate with the interdisciplinary team to address complex social and discharge planning issues while working with community agencies to meet patient needs across the continuum of care. Maintain full access to patient health information during these interactions. Requires a Baccalaureate Degree in Social Work, with a Master's preferred, along with relevant licensing and at least one year of hospital experience. Must possess full range of body motion to pass a basic FIT test, including the ability to lift and carry up to 30 pounds.
Required Qualifications
- Baccalaureate Degree in Social Work
- Must have working knowledge of community social and health agencies
- Must have knowledge of human behavior and personality dynamics
- Must possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time
- Must be able to lift and carry up to 30 pounds
Desired Qualifications
- Master's in Social Work
- Master's in related field
- License or certification in Social Work
- At least one year of hospital social work experience
- Understanding of principles and practice of social work
