Medical Social Worker -Full time

Provide clinical social evaluations and develop total plans of care with multidisciplinary clinicians for patients receiving skilled nursing and rehabilitative care in their homes. Identify community resource referrals to address practical and environmental needs for clients and families, while acting as an advocate to navigate the community system. Develop and maintain contracts with public and private agencies, then educate patients and family members on resources to promote health, safety, and independent living. Accurately document observations, interventions, and evaluations pertaining to patient care management. Requires a Master of Social Work with at least one year in a health-related field, experience with electronic medical record software, and licensure where applicable.
