Medical Social Worker - Full Time, 36 hours per week - Bronson Family Medicine, The Groves - Kalamazoo

Medical Social Worker in Bronson Family Medicine (Kalamazoo, MI) will deliver care management and behavioral health services in an ambulatory primary care setting. Responsibilities include coordinating care to connect patients and families with community services, administering behavioral health screenings and assessments, developing patient-centered care plans with an interdisciplinary team, delivering brief interventions for lifestyle/behavior changes, crisis response in the ambulatory setting, identifying psychosocial needs, and collaborating with the care team to optimize health outcomes. Requires Master of Social Work (MSW) with LMSW in Michigan; experience in medical/mental health settings preferred; strong communication, autonomous work capability, leadership, and ability to mentor others. Shift is Full Time (36 hours/week) with potential for training, process improvement involvement, and documentation in patient records. Location is Kalamazoo, MI.
