Job Summary
Medical Social Worker at Oak Street Health providing assessments, advocacy, and comprehensive care coordination for high-risk patients and caregivers. Responsibilities include comprehensive assessments, telephonic check-ins, in-person visits, goal identification with patients and support persons, referrals to community resources, navigating insurance benefits with the PRM, longitudinal and episodic care coordination, education on supportive care options and goals of care, psychoeducation for patients and families (e.g., dementia, aging, caregiver support), collaboration with care teams to develop creative solutions, timely documentation, supervision/consultation with Community Health Workers, crisis response, and other duties as assigned. Requires an MSW, licensure, language fluency where needed, and US work authorization. Anticipated 40 hours per week; full-time position with competitive benefits and pay range $51,256 - $111,351, plus potential bonus.
Required Qualifications
- Master of Social Work degree required from a CSWE-accredited school
- Licensed in the state where OSH Center is located
- Fluency in Spanish, Polish, Russian, or other languages spoken by communities served (where necessary)
- Proficient computer skills (MS Office, GSuite, EMR)
- US work authorization
