Job Summary
Medical Social Worker – Home Health role focused on assessing patients’ psychosocial status and environment, developing and implementing social service intervention plans, documenting clinically, and coordinating with the interdisciplinary care team. Responsibilities include communicating findings to the Registered Nurse, serving as a liaison between patients, families, and community agencies, facilitating case conferences and discharge planning, and connecting patients with community resources. Requires an MSW and an LCSW license, with at least 1 year of healthcare experience and current CPR certification. Per diem schedule with Monday–Friday day options and the ability to choose hours; home health setting in Pompano Beach, FL.
Required Qualifications
- Master of Social Work from a program accredited by the Council on Social Work Education
- Licensed Clinical Social Worker (LCSW) required
- Minimum of one (1) year of healthcare experience
- Home health experience preferred
- Strong verbal and written communication skills
- Excellent organizational skills
- Current CPR certification
