Job Summary
Assess each patient's unique condition by combining data from medical diagnoses, personal interviews, and professional knowledge to establish an accurate picture of overall mental and emotional health. Make routine visits to assess residents' psychosocial status, communicate with physicians regarding progress and concerns, and maintain accurate clinical and progress notes. Work closely with the health care team to develop treatment and discharge plans based on psychosocial assessments and accepted social work interventions. Develop relationships with community resource groups, represent the organization at meetings and committees, and conduct monthly Resident Council Meetings in group homes and skilled nursing facilities. Assist with obtaining school-based program components and help parents create advance directives and guardianship plans. Participate in planning and implementing quality improvement activities while adhering to department policies.
Required Qualifications
- Master's Degree in Social Work
- 1-2 years' experience working with families and children in a healthcare environment
- Valid Florida Driver's License
- CPR and First Aid Certification
- HIV/AIDS Certification
Desired Qualifications
- Experience with the Adult Persons with Disability (APD)
- Experience with Medicaid and Social Security processes and requirements
