Job Summary
Provide in-home social work services as a Medical Social Worker (MSW) to patients in their residence, performing bio-psychosocial assessments, developing and implementing treatment plans, and providing crisis intervention and counseling under supervision. Responsible for documenting assessments and progress notes, interpreting a patients condition, and coordinating Plan of Care with the care team to promote quality of life. Requires CSWE-accredited MSW/Doctoral degree and at least two years of home health social work experience; role includes travel to patient homes and flexible scheduling.
Required Qualifications
- Masters or Doctoral Degree from a school of social work accredited by the Council on Social Work Education
- Two (2) years of Social Work experience in a Home Health setting
