Job Summary
Provides psychosocial assessment and discharge planning for patients and families, collaborates with a multidisciplinary health care team to coordinate post-acute care, palliative/hospice services, and community resources, advocates for patient involvement in care planning, educates patients and families on advance directives and resources, and maintains knowledge of relevant regulations; requires MSW with licensure eligibility (IL-LSW) or LCSW preferred, and 1 year healthcare social work experience; travel may be required; 40 hours/week, full-time.
Required Qualifications

- Master’s degree in social work from a CSWE-accredited school
- 1 year of social work experience in healthcare
