Job Summary
Medical Social Worker responsible for delivering social work and care management services across hospital and ambulatory settings, including crisis intervention, discharge planning, advocacy, and coordination with interdisciplinary teams to support patients and families across pediatric to geriatric populations. Roles cover assessment, community resources, behavioral health screenings, brief interventions, and leadership in palliative and end-of-life/social work pathways. Requires a Master’s in Social Work and appropriate licensure, with licensure expected within two years of hire; HIM and patient-centered collaboration emphasized.
Required Qualifications
- Master’s Degree in Social Work
- Licensed Master’s Social Worker (LMSW) or Limited License MSW (LLMSW) in good standing OR Master’s Degree in Psychology or Counseling Psychology and Licensed Professional Counselor (LPC) or Limited License Professional Counselor (LLPC) or License Psychologist (LP) or Limited License Psychologist (LLP) will be considered
- Fully licensed within two years of hire including state required supervision hours and licensing exam
- 1-2 years professional level of work experience in healthcare or mental health, preferred
- Current driver’s license in the state of Michigan and proof of insurance required
