Medical Social Worker (58323)

Provide home-based social services to patients dealing with depression, anxiety, poor nutrition, or a lack of resources. Monitor psychosocial conditions, conduct assessments, document progress, and ensure patients move toward individual goals. Deliver counseling, community resource planning, crisis intervention, and advocacy while consulting with family and caregivers on care plans. Work within a home health team including RNs, LPNs, and therapists focused on the patient's plan of care. Requires a Master's degree in Social Work, active Wisconsin license, and one year of experience.
