Job Summary
Accompany healthcare providers during patient visits to document medical histories, physical examinations, diagnostic tests, and treatment plans in real-time within electronic health record systems. Enter accurate ICD-10 and CPT codes for diagnoses and procedures, then serve as a liaison between providers and the coding team to resolve documentation queries. Regularly review patient charts for completeness and accuracy, conduct research for morning huddles, and oversee quality metrics to identify care gaps. Analyze population health data to stratify at-risk patients and assist with chronic care management enrollment while staying updated on evidence-based protocols. Adhere strictly to HIPAA regulations and maintain confidentiality for all patient data.
Required Qualifications
- Minimum of 2 years of clinical experience
- Ability to efficiently use electronic health record (EHR) systems
- Strong typing skills and advanced computer proficiency
- Effective written and verbal communication skills
- Ability to thrive in a fast-paced clinical environment with a commitment to accuracy
- Strong organizational skills, effective time management, and problem-solving capabilities
- Willingness to move between Neighborhood Centers as needed
- Proficiency in both English and Spanish
Desired Qualifications
- Bachelor's degree in healthcare or related field, preferred
- Familiarity with medical terminology, anatomy, and physiology
- Prior experience as a Medical Scribe
