Accompany the physician into the patient examination room to accurately document the encounter and others present. Chart patient encounters in the electronic medical record system, including history, physical exams, diagnostic findings, lab results, consultations, treatment courses, diagnoses, and follow-up instructions. Review primary diagnoses with the provider to ensure documentation is accurate and clinically supported. Enter proposed orders with accurate diagnosis codes per provider direction and facilitate the acquisition of test results prior to encounters. Continually check the status of pending studies to increase department efficiency and document any procedures performed.
