Job Summary
Medical Scribe/Clinical Documentation Specialist responsible for documenting patient encounters in the EHR, entering histories, assessments, treatment plans, diagnoses, and progress notes; ensure accuracy, completeness, and compliance with HIPAA and agency standards; track unsigned notes and authorization requirements; maintain confidentiality; communicate with psychiatrists, therapists, case managers, nurses, and administrative staff; participate in staff meetings and quality improvement activities; ensure documentation meets Medicaid, Medicare, and accreditation standards; role supported by experience in medical scribing, behavioral health, medical records, or clinical documentation; familiarity with EHR systems such as Epic, Credible, eClinicalWorks; strong typing, grammar, proofreading, and organizational skills; bilingual English/Spanish preferred in Miami-Dade settings; work environment includes office and clinical behavioral health setting with hybrid or in-person schedules possible.
Required Qualifications
- High school diploma or GED required; associate or bachelor’s degree preferred.
