Job Summary
Process referrals for OBGYN and Family Practice, facilitating patient care follow-up and acting as the direct contact between GFHN clinicians and referring providers. Interface with patients, medical staff, nursing staff, patient accounts, and agencies to resolve referral system issues, while case managing complex referrals with clinician guidance. Collaborate with clinic management, nursing, and clerical staff within the Patient Centered Medical Home to maintain high standards of patient care. Requires a Medical Assistant Certificate, CPR certification, bilingual English-Spanish skills, and a valid CA driver's license. Full-time 40-hour week at Gardner Health Services' CompreCare Health Center in San Jose.
Required Qualifications
- High School graduate or equivalent
- Medical Assistant Certificate from an approved Medical Assistant program as required by the State of California
- CPR certified
- Bilingual skills in English-Spanish
- Possession of a valid CA driver's license
