Job Summary

Sort, scan, and file patient health information records including lab reports, correspondence, and referrals into the EHR while examining files for completeness and compliance with Joint Commission standards. Process incoming reports, manage release of information requests, and coordinate documentation with hospitals and external organizations to track progress of requested materials. Maintain the PHI Log, abstract data for insurance and government agencies, and prepare statistical reports to assist health service operations. This role involves covering any San Diego PACE facility location with up to 30% travel, working days, evenings, and weekends as needed.

Required Qualifications

  • 1 year of experience in medical records setting
  • Strong attention to detail
  • Acute care experience
  • Clinical experience in area of specialty
  • High School Diploma or equivalent
  • CA driver's license with appropriate insurance coverage
  • Excellent critical thinking skills
  • Knowledge of team building skills and principles of self-managed work teams
  • Strong problem-solving abilities
  • Highly detail-oriented with a careful eye for quality control and presentation of work
  • Strong verbal communication skills
  • Strong written communication skills
  • Good customer service skills
  • Experience in discerning discrepancies or deficiencies and preparing written notifications for corrective action concerning patient records
  • Knowledgeable concerning the release of information under various methods including the FOIA and Privacy Act
  • Experience in developing various statistical reports to assist health service operations
  • Experience in retrieving information, conducting research, and analyzing records for completeness and compliance with standards
  • Experience in coordinating documentation and recording information with hospitals, other treatment facilities, or other outside organizations
  • Experience in tracking the progress of requested materials
  • Skilled in conflict management and resolution
  • Knowledge of medical terminology
  • Basic computer skills
  • Ability to proficiently maneuver through all electronic applications used for the job
  • Must type 35 WPM
