Job Summary
Licensed Practical Nurse (LPN) position providing in-home direct patient care per the provider’s plan of care, including vitals monitoring, medication administration (including insulin and IV/fluids), wound care and dressing changes, and assessment of patient conditions. Collaborates with providers and care team to ensure quality outcomes. Flexible scheduling (Full-time, Part-time, PRN) with in-home visits at two Pennsylvania locations (Johnstown and South Fork); shifts include daytime or overnight. Prioritize patient safety, documentation, and adherence to nursing scope of practice. Also requires graduate nursing program completion and current LPN license with CPR/BLS/First Aid certification; reliable transportation to care sites.
Required Qualifications
- Graduate of an accredited Nursing program
- Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
