Job Summary
Deliver comprehensive primary care across the lifespan in a Hawaii-based community health center. Roles include Nurse Practitioner (NP), Physician Assistant (PA), Pediatrician, or Physician (Family Practice or Internal Medicine). Provide assessments, diagnoses, treatment planning, prescribing, patient education, and referral to specialists as needed within an integrated care team. Maintain timely documentation in the EHR (within 48 hours, preferably same day); participate in quality improvement and preventive care protocols; supervise learners as appropriate; and support care coordination and patient advocacy while working in an outpatient clinic setting (inpatient, obstetrics, and admitting privileges not included).
Required Qualifications
- Current MD, DO, NP, or PA credentials
- Eligible for credentialing in the State of Hawaiʻi
- Experience working in a community health center or with underserved, low-income populations desirable
