Job Summary
Provide high-quality, person-centered physical therapy to senior residents in a skilled nursing setting. Assess, plan, and deliver individualized PT treatment to improve mobility, function, and quality of life. Partner with an interdisciplinary team, educate residents and families, document progress, and contribute innovative ideas that support seniors’ independence and overall well-being in our skilled nursing community.
Required Qualifications
- Licensed Physical Therapist (PT) or eligible for licensure in Colorado
- Ability to develop and implement individualized treatment plans
- Strong assessment and documentation skills
- Experience with geriatrics and skilled nursing preferred
- Interdisciplinary teamwork and family education skills
