Job Summary
Physical Therapist to provide in-home outpatient rehabilitation services for older adults, evaluating and treating patients in their homes or in independent living facilities, designing and delivering individualized therapy programs to restore function and promote independence. Responsibilities include conducting assessments, developing plans of care, coordinating with the rehabilitation team, incorporating therapeutic exercises (gait, balance, strength, flexibility), manual therapy, neuromotor and functional training, and educating patients and caregivers. Must hold a valid PT license in the practicing state(s) and have a Master’s degree in Physical Therapy (or equivalent PT-related advanced degree) with eligibility for licensure. The role supports visa sponsorship (H-1B) and relocation assistance, and offers mentorship, flexible scheduling, and potential supervision of PT assistants. The position emphasizes home-based care, in-home evaluations, and interdisciplinary collaboration to optimize patient outcomes.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice
- Master’s degree in Physical Therapy (or Master’s in PT-related field in addition to a Bachelor’s degree in Physical Therapy)
- Completion of required licensure/border qualifications as applicable by state
- Ability to provide in-home/outpatient rehabilitation services
- Ability to supervise PTAs and students as applicable by state licensure requirements
- Experience in outpatient rehab, geriatrics, neuromuscular therapy, or home health settings preferred
