Job Summary

Assess patients' strengths and weaknesses to devise creative treatment plans and execute daily plans of care. Collect and record patient care data using Prompt EMR while overseeing support staff including PTAs and aides. Provide education and training on ADLs, therapeutic activities, and community reintegration, identifying needs for consultations or referrals to other healthcare professionals. Prepare evaluations in accordance with department policies and standards of practice. Promote JAG in the community through lectures and presentations. Maintain patient confidentiality and operate in a high-energy environment focused on healing and functional independence.

Required Qualifications

  • Bachelor's or Master's Degree
  • Licensed Physical Therapist in the state practicing in

  • Doctorate degree
  • Experience/knowledge with an EMR software
  • Basic computer skills
  • Works as a team member and individually with minimal supervision
  • reflects an understanding of reimbursement systems and their impact on patient care
  • obtaining patient history
  • exhibiting knowledge of patient diagnoses
  • researching unfamiliar diagnoses
  • Provides education/training to patient and/or patient support system
  • Training-This includes but is not limited to ADLs/IADLs, therapeutic activities (e.g. transfers, bed mobility), rehabilitation equipment prescription and training and community reintegration
  • Provide data, note writing using Prompt EMR for all treatments provided
  • Oversee support staff of PTA, ATC, PT aides
  • Attends annual JAG Billing and Coding seminar
  • Promotes JAG in the community through lectures/presentations
