Job Summary
Deliver skilled physical therapy to patients based on individualized Plans of Treatment developed with referring physicians. Evaluate needs to set rehabilitation goals for mobility, strength, and balance, then collaborate with interdisciplinary teams including nurses and occupational therapists. Document all visits, assessments, and progress notes accurately. Support patients in maximizing independence and improving quality of life in their homes. Requires Michigan PT licensure, a Master's degree, and at least one year of experience in hospital, home health, or outpatient rehab settings. Full-time role in Midland, MI covering the Saginaw territory with a fleet car program and $7,500 sign-on bonus.
Required Qualifications
- PT Licensure - Michigan
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
