Job Summary
Deliver skilled physical therapy to patients based on individualized Plans of Treatment, evaluating needs to improve mobility, strength, and balance. Collaborate with interdisciplinary home health teams including nurses and therapists to provide coordinated care. Document all visits, assessments, and progress notes accurately and in a timely manner. Support patients in maximizing their independence and improving quality of life in their homes. Requires Michigan PT licensure, a Master's degree, and one year of experience in hospital, home health, or outpatient rehab settings. Full-time role in Midland, MI covering Saginaw territory with a $7,500 sign-on bonus and fleet car program available.
Required Qualifications
- PT Licensure - Michigan
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
Desired Qualifications
- ~ In-home therapy delivery
- ~ Geriatric rehab, orthopedic or neurologic rehab
- ~ Post-acute care or chronic condition management
- ~ Working as part of a mobile or interdisciplinary care team
