Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed with referring physicians. Evaluate patient needs, set rehabilitation goals, and collaborate with interdisciplinary home health teams including nurses and occupational therapists to provide coordinated care. Document all visits, assessments, and progress notes accurately. Support patients in maximizing independence and improving quality of life within their homes. Requires Michigan PT licensure, a Master's degree, and one year of experience in hospital, home health, or outpatient rehab settings. Covers Saginaw territory with a fleet car program available.
Required Qualifications
- PT Licensure - Michigan
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
