Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs to improve mobility, strength, balance, and overall physical function, then document all visits, assessments, and progress notes accurately. Collaborate with an interdisciplinary home health team including nurses, occupational therapists, and social workers to provide coordinated care. Manage in-home therapy delivery for geriatric, orthopedic, or neurologic rehab within a mobile care team setting. Requires a Master's degree in Physical Therapy and current Indiana licensure.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- Physical Therapy - Licensure in IN
Desired Qualifications
