Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs to improve mobility, strength, and balance while documenting all visits and progress notes accurately. Collaborate with the interdisciplinary home health team, including nurses and occupational therapists, to provide coordinated care for geriatric, orthopedic, and neurologic rehab cases. Support patients in maximizing their independence and improving quality of life within the comfort of their homes as part of a mobile care team.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- Physical Therapy - Licensure in IN
- Schedule: Monday-Friday, 8am-5pm
Desired Qualifications

- ~ Working as part of a mobile or interdisciplinary care team
