Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs to improve mobility, strength, balance, and overall physical function while collaborating with the interdisciplinary home health team. Document all visits, assessments, and progress notes accurately and in a timely manner. Support patients in maximizing their independence and improving quality of life within their homes. Work Monday through Friday from 8am to 5pm in a full-time capacity. A Master's degree in Physical Therapy and current licensure in Indiana are required, along with one year of experience in a hospital, home health, or outpatient rehab setting.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- Physical Therapy - Licensure in IN
- Schedule: Monday-Friday, 8am-5pm
