Job Summary
Deliver skilled physical therapy to patients based on individualized Plans of Treatment developed with referring physicians. Evaluate patient needs to set rehabilitation goals that improve mobility, strength, balance, and overall function. Collaborate with the interdisciplinary home health team, including nurses and therapists, to provide coordinated care. Document all visits, assessments, and progress notes accurately and in a timely manner. Support patients in maximizing their independence and improving quality of life at home. This role requires a Master's degree in Physical Therapy, current state licensure, and at least one year of experience in hospital, home health, or outpatient rehab settings. Candidates must hold a valid driver's license and reliable transportation. Schedule is Monday-Friday, 8am-5pm, with a $5,000 sign-on bonus.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
Desired Qualifications

- ~ Post-acute care or chronic condition management
