Job Summary
Deliver skilled physical therapy to patients based on individualized Plans of Treatment developed with referring physicians. Evaluate patient needs to establish rehabilitation goals that improve mobility, strength, balance, and overall physical function. Collaborate with interdisciplinary home health teams including nurses, occupational therapists, and social workers to provide coordinated care. Document all visits, assessments, and progress notes accurately and in a timely manner. Support patients in maximizing independence and improving quality of life within their homes. Requires Michigan PT licensure, a Master's degree, and one year of experience in hospital, home health, or outpatient rehab settings.
Required Qualifications
- PT Licensure - Michigan
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
Desired Qualifications
- ~ In-home therapy delivery
- ~ Geriatric rehab, orthopedic or neurologic rehab
- ~ Post-acute care or chronic condition management
- ~ Working as part of a mobile or interdisciplinary care team
