Job Summary
Deliver skilled physical therapy to patients based on individualized Plans of Treatment developed with referring physicians. Evaluate patient needs to establish rehabilitation goals improving mobility, strength, balance, and overall function. Collaborate with the interdisciplinary home health team including nurses and occupational therapists to provide coordinated care. Document all visits, assessments, and progress notes accurately and in a timely manner. Support patients in maximizing independence and improving quality of life within their homes. Requires a Master's degree in Physical Therapy, current state licensure, and 1+ year of experience in hospital or home health settings. Schedule is Monday-Friday, 8am-5pm.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- Physical Therapy - Licensure in IN
- Schedule: Monday-Friday, 8am-5pm
