Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs to improve mobility, strength, balance, and overall physical function while collaborating with nurses, occupational therapists, and social workers for coordinated care. Document all visits, assessments, and progress notes accurately. Support patients in maximizing independence and improving quality of life in their homes. Requires Michigan PT licensure, a Master's degree, and 1+ year of experience in hospital, home health, or outpatient rehab settings. Includes geriatric, orthopedic, and neurologic rehab focus with a fleet car program available.
Required Qualifications
- PT Licensure - Michigan
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
Desired Qualifications
- ~ In-home therapy delivery
- ~ Geriatric rehab, orthopedic or neurologic rehab
- ~ Post-acute care or chronic condition management
- ~ Working as part of a mobile or interdisciplinary care team
