Job Summary
Deliver skilled physical therapy to patients based on individualized Plans of Treatment, evaluating needs and developing rehabilitation goals to improve mobility, strength, balance, and overall function. Collaborate with the interdisciplinary home health team, including nurses and therapists, to provide coordinated care and document all visits, assessments, and progress notes accurately. Support patients in maximizing independence and improving quality of life within their homes. Requires Michigan PT licensure, a Master's degree, and 1+ year of experience in hospital, home health, or outpatient rehab settings. Full-time role covering Saginaw territory with a $7,500 sign-on bonus, fleet car program, and comprehensive benefits including medical, dental, vision, and 401k match.
Required Qualifications
- PT Licensure - Michigan
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
