Job Summary
Deliver skilled physical therapy to patients based on individualized Plans of Treatment developed with referring physicians. Evaluate needs, set rehabilitation goals to improve mobility and function, and collaborate with interdisciplinary teams including nurses and therapists. Document all visits, assessments, and progress notes accurately. Support patients in maximizing independence and improving quality of life in their homes. Requires Michigan PT licensure, Master's degree, and 1+ year of experience in home health or rehab settings. Full-time role in Midland, MI with a $7,500 sign-on bonus and fleet car program.
Required Qualifications
- PT Licensure - Michigan
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
Desired Qualifications
- ~ In-home therapy delivery
- ~ Geriatric rehab, orthopedic or neurologic rehab
- ~ Post-acute care or chronic condition management
- ~ Working as part of a mobile or interdisciplinary care team
