Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs to improve mobility, strength, balance, and overall function while collaborating with nurses, occupational therapists, and social workers for coordinated care. Document all visits, assessments, and progress notes accurately. Support patients in maximizing independence within their homes, with a focus on geriatric, orthopedic, and neurologic rehab in post-acute or chronic settings. Requires a Master's degree, Michigan PT licensure, and valid driver's license. Full-time role in Midland, MI with a $7,500 sign-on bonus and fleet car program.
Required Qualifications
- PT Licensure - Michigan
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
Desired Qualifications
