Medical (Physiotherapist)

Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function. Collaborate with the interdisciplinary home health team, including nurses, occupational therapists, and social workers, to provide coordinated care. Document all visits, assessments, and progress notes accurately and in a timely manner. Support patients in maximizing their independence and improving quality of life in their homes. Requires a Master's degree in Physical Therapy, current Indiana licensure, and one year of experience in hospital, home health, or outpatient rehab settings.
