Job Summary
Deliver skilled physical therapy to patients based on individualized Plans of Treatment, evaluating needs to improve mobility, strength, balance, and overall function. Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care. Document all visits, assessments, and progress notes accurately and in a timely manner. Support patients in maximizing their independence and improving quality of life at home. Requires a Master's degree in Physical Therapy, current Indiana licensure, and 1+ year of experience in hospital, home health, or outpatient rehab settings. Schedule is Monday-Friday, 8am-5pm.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- Physical Therapy - Licensure in IN
- Schedule: Monday-Friday, 8am-5pm
